Why India and South Asia's cut-points look the way they do
The Misra 2009 consensus for Asian Indians set overweight from BMI 23.0 and obesity from 25.0 — matching the WHO Asia-Pacific scale. The detail most charts miss is the lower bound: the consensus defines the normal range as starting at 18.0, not 18.5.
That reflects the same underlying finding from the other end of the scale. Indian body composition studies found optimal BMI cut-points clustering between 19 and 24 in men and 21.5 and 24 in women, with a lower underweight boundary than the European-derived 18.5.
South Asians are also the population where the "normal-weight but high-risk" pattern is most pronounced. A person can sit at BMI 22 — squarely normal on every chart — and still be at raised metabolic risk if their waist exceeds 90 cm (men) or 80 cm (women). The term used for this in the literature is the thin-fat phenotype.
In January 2025 a 115-member India Obesity Commission published a revision that lowers the diagnostic line further: increased adiposity above BMI 23, and obesity at BMI above 23 when combined with a waist of 90 cm or more (men) / 80 cm or more (women), or a waist-to-height ratio above 0.5. Neither ICMR nor the health ministry has formally endorsed it yet, so you may see clinicians using either framework.

A BMI of 25.0 is classified as Obese in India and South Asia (Obese). The same value reads differently on the WHO international scale — which is why the scale has to be named.
Waist circumference and central obesity
Abdominal obesity is defined as a waist of 90 cm or more in men and 80 cm or more in women — the lowest female threshold of any standard on this page.
Waist matters because BMI cannot see where fat sits. Two people with the same BMI can have very different visceral fat, and visceral fat is the component most strongly linked to type 2 diabetes and cardiovascular disease. The ethnicity-independent screen is waist-to-height ratio: keep your waist below half your height.

Frequently asked questions
- What is a healthy BMI for an Indian adult?
- 18.0 to 22.9 under the 2009 consensus statement for Asian Indians. BMI 23.0–24.9 is overweight and 25.0 or above is obese. Note that the WHO international chart would call BMI 24 "normal" — for Indian adults it is not.
- My BMI is normal but my waist is 95 cm. Should I be concerned?
- Yes. South Asians are the population where abdominal fat predicts metabolic risk most independently of BMI. A waist above 90 cm in men or 80 cm in women is defined as abdominal obesity in the Indian consensus regardless of what the BMI says.
- Why is the Indian underweight threshold 18.0 instead of 18.5?
- Because the Indian consensus derived its range from South Asian body composition data rather than importing the European figures. The studies behind it placed optimal BMI cut-points in the 19–24 range for men.
