Keep your waist under half your height — a ratio below 0.5. UK NICE guideline NG246 (2022) classifies 0.4–0.49 as healthy central adiposity, 0.5–0.59 as increased and 0.6 or above as high, and states the classification applies to both sexes, all ethnicities and people with a BMI under 35. The ADA 2026 adult obesity standards go further: a ratio of 0.5 or more can trigger an obesity classification on its own, regardless of BMI.

Why waist-to-height ratio matters more than BMI for Asian populations

BMI is calculated from two numbers — height and weight — so it cannot distinguish muscle from fat, and it cannot see where fat is stored. Both limitations bite harder in Asian populations: at the same BMI, people of Asian descent carry a higher proportion of body fat, and more of it viscerally rather than under the skin.

The ADA quantified this in its 2026 adult obesity standards: BMI's sensitivity for detecting obesity is roughly 50%, with specificity around 90%. In plain terms, about half of the people who have obesity are missed by BMI alone. The guideline names Asian populations and older adults as the groups where BMI most often understates body fat.

Waist-to-height ratio does one thing BMI does not: it puts height in the denominator, which makes it scale naturally across body sizes. That is why it needs no ethnicity-, sex- or age-specific cut-points — the property a 2010 systematic review identified as its main advantage over other anthropometric indices.

The trade-off is that it only reflects central fat; it says nothing about whether your overall weight is too low. Read the two together: BMI for the whole picture, waist-to-height ratio for where the fat actually sits.

How to measure your waist correctly

  1. Measure at the midpoint between your lowest rib and the top of your hip bone. Do not measure at the waistband of your trousers — that sits lower than your actual waist.
  2. Measure against bare skin, or over a single thin layer. Thick clothing adds centimetres that are not there.
  3. Stand relaxed with your feet slightly apart and your weight evenly distributed.
  4. Read the tape at the end of a normal breath out. Do not hold your breath, and do not suck in.
  5. Keep the tape horizontal all the way round, parallel to the floor — no twisting, no slipping down at the back.
  6. Do not pull it tight. The tape should sit flat against the skin without compressing it. Measure twice and average; the two readings should agree within 1 cm.

Waist-to-height ratio is a universal threshold, but national health authorities still publish their own absolute waist figures. These come from their own documents — note that the female threshold ranges from 80 cm to 85 cm.

Common mistakes

  • Measuring at the trouser waistband. For most people that sits 2–5 cm below the anatomical waist and understates the result.
  • Sucking in or holding your breath. This depresses the reading and makes repeat measurements unstable.
  • Applying one waist number to every height. A 90 cm waist on a 190 cm frame and on a 155 cm frame are not the same thing — which is exactly the problem waist-to-height ratio solves.
  • Stopping at a normal BMI. A BMI of 22 with an 88 cm waist is normal-weight central obesity, and it is common in Asian populations.
  • Using adult thresholds on children. NICE suggests 0.5 applies to children aged 5 and over as well, but paediatric assessment should still lead with age- and sex-specific percentiles.
  • Using it during pregnancy. Waist circumference changes with the uterus and amniotic fluid; adult central-adiposity classifications do not apply.

Why does waist-to-height ratio matter more than BMI for Asian people?
Because BMI cannot tell muscle from fat or show where fat sits. At the same BMI, people of Asian descent tend to carry more of their fat viscerally, around the abdominal organs — the pattern most strongly linked to type 2 diabetes and cardiovascular disease.
